Which form do I choose?

·         Account Creation – Debtors:  Please complete this form if your publications are to be distributed as a free product.  This provides us with the information to invoice distribution and warehousing charges.

·         Account Creation – Creditors:  Please complete this form if your publications are saleable products.  This provides us with the financial information to reimburse sale proceeds and to invoice distribution and warehousing charges.

Please return the completed form to bookshop@sds.qld.gov.au 

We will advise you of your account number which is to be included on the remaining forms and for future invoice correspondence.

No products can be added to The Queensland Government Bookshop website until an account has been created.
